Hospice Aides

The hospice aide may educate family members and other caregivers on patient care so that the families will feel comfortable providing day to day care to the patient between hospice aide visits.

Palliative Care is a type of care focused on providing comfort and promoting quality of life. It is aimed to alleviate symptoms as well as providing the support needed to meet spiritual and emotional needs.

Respite care is intended to allow the caregivers, often the spouse, children or family member, to take some time away from the patient, and even allow the patient some time away from the caregiver.

Continuous Care

Continuous care is utilized to provide short-term crisis management, similar to General Inpatient Care. This around-the-clock care type is provided by a licensed nurse, in any setting that is considered home.

OUR TEAM

Nurses

Each patient is assigned a primary nurse, who functions as a case manager. The nurse provides ongoing support and education for the patient and family, while coordinating pain and symptom control under the direction of the patient's physician. Many of our nurses are certified in hospice and palliative care.

Volunteers

Hospice volunteers are cherished members of the Hospice team. They are extensively trained and provide services that are extremely important in assisting the patient and family through this difficult period. They may be involved in direct patient care, office help, special projects and bereavement services.

Certified Nursing Assistants

Certified Nursing Assistants work closely under the supervision of the Registered Nurses, providing personal care and light housekeeping for our patients and families.

Bereavement Coordinators

Bereavement Coordinators are available for support to the patients and families upon admission to the hospice program, and continue to provide support throughout the 13-month bereavement program.

Pastoral Care

Hospice volunteers are cherished members of the Hospice team. They are extensively trained and provide services that are extremely important in assisting the patient and family through this difficult period.

Social Workers

Medical social workers work closely with patients and family members who are experiencing mental, emotional, family and/or financial stress due to their or their loved one’s medical condition.

We believe in the importance of meaningful relationships, the value of dignity and respect and cherish the ability to provide exceptional care and support to every individual at every stage of life.

With the help and support of the hospice care team, the patient and their family can attain physical, emotional, and spiritual preparation for the end of life.

Hospice care neither prolongs life nor hastens death, but relieves pain and physical discomfort so that the patient can experience a more peaceful and satisfying state.

We empower our patients and encourage them to share their concerns, thoughts and wishes so we can better understand their desires and strive to meet their unique needs.
